The IMON center in New Britain supports pregnant patients to ensure a healthy pregnancy and birth at all stages of pregnancy, including the critical time after delivery, often called the fourth trimester. This important time after birth is when new parents need additional support as they transition into caregivers for a newborn and their bodies are recovering–and when complications and challenges can arise.
The IMON Center is committed to supporting each of its pregnant patients and their identified support network through this special time by delivering sensitive, respectful, culturally and linguistically appropriate care at the highest levels before, during and after delivery though a pregnancy-centered model. The Center will connect patients in a group format to provide culturally-informed care to build patient trust to combat maternal mortality and morbidity rates among women of color, particularly African American women. This team will partner with Hartford Health Care (THOCC) for consultation, referral for high risk patients, and labor and delivery. IMON Center patients will be connected to Family Wellness Center.
While IMON Center patients will be able to contacts CHC’s general patient support lines, the use of the Pomelo Care app will help ensure that patients have a live resource 24/7 for support on any medical need outside of usual operation hours, if new mothers have questions about their baby’s care, breastfeeding, physical symptoms or challenging emotions they themselves might be experiencing.
