What does it mean to be a Patient-Centered Medical Home?
Simply put, it means our patients–you–are partners with the CHC medical team for your own medical care. Using up to date data and research (evidence-based care), our compassionate medical care team, including doctors, nurses, medical assistants and many others, are committed to helping you manage your health.
What does that mean for patients?
As the patient, you play the most important role! In the Patient-Centered Medical Home, patients are encouraged to:
- Actively participate in their healthcare by asking questions, expressing concerns, and providing information to your healthcare team about your medical history and symptoms.
- Communicate openly with your healthcare providers about you health, preferences, and any barriers they may face in following medical advice.
- Work with your providers to make decisions about your care, taking into account the your values, preferences, and goals.
- Take advantage of health resources and information to help understand health conditions, treatment options, and preventive measures, so you can make informed decisions about your healthcare. This includes asking your care team questions or for more information.
- Participate in preventive care measures, such as screenings, vaccinations, and routine check-ups, to maintain your overall health and catch potential issues early.
- Coordinate care, especially if you have multiple healthcare providers. This involves sharing information between different healthcare professionals to ensure comprehensive and integrated care.
- Provide feedback about your experience, to help us continually improve the quality of care, and come up with new ways to provide services to you and other patients.

What is CHC’s role in the Patient-Centered Medical Home?
Since 1972, Community Health Center, Inc. has been building a world-class primary health care system committed to excellence, focused on improving health outcomes and building healthy communities. We use the latest technology to coordinate care and address each patient’s unique needs. Our goal is to have long-term, trustworthy relationships with all of our patients.
As part of our services:
- We use medical interpreter services.
- We use electronic health records for all patients
- Patients can easily view their own records online
- We can help you apply for state medical insurance
- We have a sliding fee scale – all patients are seen regardless of ability to pay.
- Anyone can apply for the sliding fee discount; it is based on your income and family size. Please speak with a receptionist or Access to Care worker for assistance with the application. You will need to present proof of income with the application.
